Near the end, we see all the bodies he pitched into the dumpster. Seems that the "doll" in his apartment was imaginary. He finally woke up and the doll was gone. The victims were just as dead in the dumpster though. The point may have been illustrating the lack of distinction between "realities" the psychotic killer has.
