When I first played through the game I hadn't realised I was supposed to get the weapons to help me fight the bosses, so I would keep trying to kill the Lich King first (Which I did after 2 or 3 tries) without having gone to any of the other stages.
They are. I have unlocked and seen these cutscenes from the game myself.
And the PSX version had both the Desert and Forest. However, only one stage of each realm made the cut. They can only be accessed after destroying Skorne in the Underworld.
If you become level 99, not only will you have 999 of a very special powerup, you will also have access to the jukebox and movie theater (where these cutscenes can be seen at.)
That would be in Gauntlet Dark Legacy. Then again, most of these cutscenes did appear in Dark Legacy.
I mentioned earlier that the Parchment would either be in the Cemetary or Spire because of the Town realm existing before it got split into the Province and Sky realms that we all know in Dark Legacy.

The PSX version didn't have the Pyramid or the Forest!

I have never seen any of these cutscenes before, and I have beaten the game several times. They had to have been from a PAL version or something.
You are right about the Desert and Forest, but I found the Sword of Dismemberment in the Ice Realm instead of the Lamp.

These legend item cutscenes rock. I wished that Gauntlet Dark Legacy had the music of these cutscenes as well.
I also wished they showed the Legendary Ice Axe similar to the way it was shown in the instruction booklet and the Dark Legacy cutscene.
BTW, the Parchment of Fire looked like it would either be in a cave at the Spire or in a Tomb at the Haunted Cemetary.
