Thats a very high tension place to put a surface piercing, especially with PTFE(i think thats what you were using), the migration rate of surface piercings is huge, you should of atleast trimmed the bar down to prevent it from catching;
Next time use surface staples and measure the skin so that the piercing sits correctly with the bar.
