The color, volume and velocity is screaming flashover. 1st floor ventilation only expedited the event. Fuel waiting to flash is too rich looking for fresh air to lean out the mixture. The vent profile should have started from the ROOF, if unable then attic window, 2nd floor windows then lastly 1st floor windows.

The smoke conditions preceeding the flashover made it pretty obvious that a flashover was about to take place. Smoke Volume: tremendous, Smoke Density: Very dense, Color: Dark Black, Velocity: Very Fast and Turbulent (sign of exothermic change--house no longer absorbing heat but reflecting it)
I wonder if there was a company inside hitting it with water. Maybe that flashover would not had happend.....but I was not there. Anyway fire is out and all went home...that is a good thing

i have studyed this fire in my classes and what happened was they did think someone was in there and they rushed it, they were also having pump problems as well, as for setting up a master stream at the door, what if there is an obstruction? or somthing that stops or deflects your stream 10 feet in?

Firefighting technology simply doesn't advance as many linear technologies do... Some cities are using fire drapes etc. whereas most still use small capacity water system, manual ventilation etc.

Middletown -- why not introduce the concept of Blitz attack? Just position a master stream nozzle at the front door, open up using the 1,000 gallons of water aboard the engine -- and voila -- you've cooled the fire down considerably and there won't be a flashover.
No good if you think there's still someone in there as this crew clearly did. They wouldn't have risked going in at all otherwise, just doused the building from the outside or let it burn down
