If that were a real ship, and you were on the way to see CDR Troi, you knew you were in deep, that your military career was probably over. Most people were probably very frightened of her, which is why she had to be so open.
It's funny, all the people who secretly wished to be on the enterprise so they could get "counseled" by troi never realized that she's bridge staff...and most likely also a department head, so regular crew would't see her normally(unless it was very serious).
On a real ship she would be the ship shrink, but couseling would be performed by your department's chain of command. After that, the chaplain, then the ship's counselor. If you ever got into her office on a real ship you had some ISSUES.

It's also obvious to me that as time went on they got better with their roles and the writing improved. Star Trek was action/adventure fodder for youngsters, middle-aged people and all in between, not an arena for serious drama.
