well, i guess it can be sometimes. i have never had to pay in italy, except in some public toilets in venice and milan. but in bars and restaurants it has always been free when i have been there... so keep some change in your pocket, but i am guessing you will be ok most of the time just going pee :) and i am a guy so it may be a bit cheaper. like at the train stations where the women do actually pay but guys only pay to "sit" if you know what i mean

If they charge your € 50, you can call the cops (Polizia or Carabinieri), they will be quite happy to fine the guy.
I suppose you meant €0,50. (fifty cents).
