The first is the sense of apathy you can fall into when you really start thinking you know too much. In the next instance he's talking about those people who think they've got everything under control. Their egoism is crazy. The last instance, perhaps most important, is him saying "I tried to be strong and good" and wondering if that in and of itself is crazy. It probably is.
