In the Game Gear version, they removed the original level 2 and replaced it with you arriving at the amusement park. Levels 2 and 3 are actually just level 3 from the Sega Genesis version, plus a couple extra sections added in the Game Gear. There was a lot of content cut from the port, not to mention Skate and especially Blaze suck due to their throws doing not much more damage than punches (and Blaze's blitz is pure garbage now).
They're listed at Streets of Rage Online; both have exclusive levels to their ports, the GG's a bit better overall though due to the inclusion of multiplayer and better graphics (the GG had a bigger palette than SMS, hence better color quality). The special attacks in GG are basically a throwback to the police specials, but it's certainly possible to beat the game without using them.
The soundtrack is actually very good considering the limitations of the GameGear. The problem is this video stutters, so the sound is not as accurate as what you would hear when playing it on an emulator.
The Gamegear is capable of doing better then this. They could have taken better advantage of the hardware. It's still an ok game but I think they were a bit lazy porting it.

Theres also a Sega Master System version for SOR/BK 1 & 2. I've only played the one for 2 and I thought it was very clunky, but I guess worth trying for the replacement levels.

Oh no, this version (Game Gear) is great, the SMS version is clunky.
But anyhow, any Streets of Rage/Bare Knuckle fan should definitely try these out.
