The one in the straight jacket was a concept for L4D that they scrapped. It was designed to scream when it saw you and it would run around and avoid you as much as it could. The reason they scrapped it was because it was too hard to kill because when it screamed it alerted the horde (just like a car alarm) and it wouldn't stop until you killed it. The thing was it alerted too many Zombies and it ended up being impossible to kill due to the endless zombies that it summoned.
