That wasn't failure to stop that was a mess load of mistakes.

#1 Was going too fast for the situation.

#2 Either wasn't paying attention or he was riding his service breaks too hard before he got to this place and his brakes had failed which leads to:

#3 He didn't get into a low enough gear before downgrade and wasn't using his engine brakes right.

#4 Plus I'm sure he didn't do a proper pre-trip inspection.
