if you listen carefully, in the intro you can hear a rattely sound. on my ipod (because of my headphones), it sounds like Portnoy is fliping the springs on his snare on and off. i think he did this to immitate the sound of someone thrashing around in a hospital bed. for those who don't know, this song is about James' daughter having a seizure. just a theory though
