I think of it like, how people don't recognize people that have problems. They still know that something is wrong, but nobody wants to try and help. Suicide is common, nobody should just expect someone to not have the guts to do it. I wouldn't say that she committed suicide. I think it was about a girl with problems the narrator was friends with, but no one really did much to help her. Due to turning a blind eye to her problems no one stopped Madeline from the dark path she took and she OD'ed.

I think he felt bad because he thought that he could have tried more and told her that she shouldn't do these things 'cause she could have turned her life around and been happy.

I think it's about a girl who didn't like her home life so she took drugs 'cause at the time it made her feel better. She talked to the guy 'cause they where mates but he wasn't there one time. He knew she wasn't happy but didn't know to what extent. Then when she got to depressed she went out and took the drugs. She either overdosed by accident or on purpose and died.
