A big head with a crooked hook = google image "opium poppy". But even without that cryptic reference, this is such a brilliant song about addiction. You say there's a finish line, you're already beat. I was in college, way too caught up with speed and coke and just starting to flirt with OC and heroin (mostly to deal with the comedown from the speed and coke).  This song really helped me straighten out before I went too far in that direction.
