....3.I'm assuming that during the incline the sciatic nerve is getting pinched in some way.It feels to me that the pain is coming from the muscle that your showing in the video.I'm feeling a muscle in the left buttock contracting and having spasms.These spasms are also occuring in my thigh and calve.I wonder if this is somethig that a chiropractor might be able to correct??
Meds just aren't working.The pain is so aweful,it's really getting to me mentally.
@Geardoodle The increase in symptoms could be due to muscular entrapment of the nerve, or it could be that the disc herniation has worsened and the nerve gets pulled up against it more with incline.It would probably be best to avoid inversion for now. If you don't get improvement, a new MRI would be a good idea to evaluate further. Good luck!
....2.The muscles in my left buttock,thigh and calve are aching to the point that I'm not sleeping well.I've been walking with a notable limp for about 10 days now.Like I said,this is an old injury,but I've never felt it with such intensity for this length of time.I got an inversion table last week to see if that would help.My problem that I'm having with the table is that when i incline to strech myself the leg pain becomes terrible....
1.I've had an ongoing problem with l5 disc herniation and sciatic pain for about 13 years now.Over the years I learned how to keep back strains to a minimum to as not to overly agrravate the pain.However,about 2 weeks ago,I woke up with unbelievable pain in my low back,and hip,and pain going down my leg.It's the same pain that I've had for years,but it's so much more severe and lasting now......
I've had a constant stiffness and sharp pain just above my left buttock since February of 2010. Standing too long, walking too long, or even sitting too long will send jolts of pain down through my left leg down into my foot making it quite uncomfortable...Prolonged exposure to cold weather also increases pain. I know I shouldn't be as worried as I am, but I'm going to be twenty in July and I should not be feeling this old at such a young age. :(
@Specialboy1 I suggest you check out my Sciatica Exercises Part 1 and Part 2 videos and try the tests/exercises on them. Your symptoms may be related to a bulging disc, or could be a less-serious problem of muscle trigger points, possibly associate with a sacroiliac joint that is not moving properly. If neither exercise is helpful, I'd suggest getting checked by a chiropractor to see if it is a sacroiliac problem. Good luck!
I have just undergone surgery at L5 S1 I've had sciatic pain for several years as well as muscle spasms from my toes curling to cramps all over left leg, above ankle outside of my leg gets the most problems constant twitching then cramps,I also have foot drop so am unable to operate my foot.Surgery was decompression with pedical screws for spondylolthesis also fusion,bone was also removed from nerves to give them more room.I have less pain in my lower back now but still have some pain in foot.
@desb63 Since you've had pain for several years, it may take up to several months for maximum recovery. It is good that you've at least had some improvement from the surgery. I wish you good luck with your recovery.
Hi doc, please answer, I'm in so much pain! Severe pain in lower back. It pulsates all day. Severe pain in left buttock that shoots down left leg. Left buttock has lost a lot of fat and is dented/flattened/misshaped where the pain is. Sitting is painful, standing is painful, walking hurts. I have disc bulge at L3-4 & L4-5 & degenerate disc changes as per MRI. I also have a huge bulge on my lower back that shows through my clothes. What should I do? Severe pain 24hours per day. I'm only 33. Help
@nikkianika I suggest you start with the exercise in my Sciatica Exercises, Part 1 video. Try to find the position that best centralizes the symptoms (explained in the video and in the free ebook available on the website shown on the video). Use that position frequently - about a minute at a time, up to 4 or 5 times per hour you're awake. Usually that will begin to improve the symptoms within a few days to a week. Hi Dr. I have sciatica symptoms-- pain runs from buttock area down to calf--that completely goes away if I sit down, but if I stand up it gets progressively worse. From what I've read, most people seem to have pain sitting down and not when they're walking, but i'm the opposite and wonder what that could mean in regards to my sciatic condition. @heyyy19 Symptoms that improve with sitting and increase with standing are usually related to either degenerative arthritic changes in the joints at the back of the spine or occasionally dysfunction in one or both sacroiliac joints. With degenerative arthritis, extra bone, thickened ligaments, and sometimes cysts on the spinal joints can narrow the space at the back of the openings where the nerves come out and standing places the spine in slight extension which further decreases the space.
@heyyy19 ...continued from previous post - If the problem is from degenerative narrowing of the nerve openings, flexion stretching exercises (such as lying face down on an exercise ball) and abdominal core strengthening maybe helpful. Other treatments like spinal decompression and massage can also relieve pressure on the nerves, and spinal injections can temporarily relieve symptoms by decreasing pressure from inflammatory swelling.
I get pain on my lower back (where the nerves come out and also the piriformis is). sometimes it's really painful specially in the cold weather. I spoke to my GP and she said with the exercise the pain would go away yet nothing's happened.
I have a big curve on my lower back as well and when I walk about 20 minutes the pain on back doubles so I must sit down or bend myself and touch my toes 3 to 5 times and it gets better. I'm a man and only 32 years old. Could you advise me what to do please?
@kataSang Since the pain gets better after touching your toes, it's probably not a disc problem. There's probably some type of dysfunction in one or both of the sacroiliac joints that is causing inflammation and muscle spasm. Sometimes, this will go away on it's own, or with stretching, but it may to be corrected with manipulation. I suggest you try the stretch shown in my Sciatica Exercises Part 2 video and if it does not help, then consult with a chiropractor or osteopath.
Injections may provide temporary relief by reducing inflammatory swelling, but will not correct the disc protrusion. The temporary reduction in symptoms may allow you to do things that can help the disc protrusion (such as the exercise on my video), so the injection may be worth considering. Whether or not you do the injection, I suggest you ask your doctor to refer you to a PT, chiropractor, or physical medicine doctor to help with pain and then for help with rehab to prevent a recurrence.
@DrGeorgeBest Thank you so much for your help. I am getting an injection today, but only out of desperation. I did not want a temporary fix, but I have not slept for a week, and I can't go on like this. I am not sure why they are so opposed to surgery unless it is not very successful. If I knew surgery (microdiscectomy) would be a permanent solution I would do that in a heart beat. I don't want this to keep coming back.
@MultiNelson69 Surgery in general doesn't have the best long-term results, especially the more aggressive types which often create problems with scar tissue development later on. Minimally invasive surgeries are usually a good option when non-surgical methods fail, but you can't really think of surgery as a "permanent" solution. You still need to be careful with your back and do preventive exercises to minimize the risk of recurrence - either at the same disc or at an adjacent one.
HELP! I have 5 MM left paracentral disc protrusion impinging on the traversing left S1 nerve root. My doc wants to send me to a radiologist for injectio. Is this the best course of action? This is probably the beginning of many problems I will have in the future. I would like a doc who will work with me on preventive measures like exercise, diet, ...whatever. I am afraid the radiologist will just do injections -not over all care. Who would be the best type of doc to see in your opinion?
I am experiencing sciatica symptoms. It feels better if I walk as opposed to resting. It always gets worse in the afternoon and I go progressively down hill from there. I had an MRI on Friday and am waiting for the results. My doctor gave me pain pills but I try to save them for the night so I can sleep. I tried swimming today and it HURT A LOT! Why does it get worse in the evening? It doesn't make any difference if I am active or rest all day,,,it always gets worse in the evening.
@MultiNelson69 In many cases, the underlying issue with increased pain late in the day is inflammatory swelling. Many people will have an increase in inflammation (regardless of activity) late in the day because their levels of the hormone cortisol will often drop at this time (this is often seen in chronic stress as the adrenal glands get overworked). I suggest you check out my Sciatica Exercises Part 1 Video and/or visit sciaticatreatmentathome . com for self-treatment tips.

Hi, I am a volleyball player and I once stretched too much behind when I was spiking. The pain has lasted for months, even now. I can't sleep well with this pain. The pain goes from my left lower back to my leg. When the pain occurs my left leg would turn all wobbly and it goes away after approximately 5-7 minutes. I wonder if it's sciatica as the symptoms are really close. Is there any way to cure it and what type of sciatica do I have?
@Leeteukluv It could be disc-related sciatica, but since the symptoms go away after a few minutes, I think it could be a problem in the sacroiliac joint. I suggest you try the exercise shown in my "Sciatica Exercises Part 1" video. If that helps, it's probably a disc and repeated use of the exercise will probably get rid of it over time. If that doesn't help, it's probably sacroiliac and I'd suggest you see a chiropractor or osteopath to get it checked and treated.
hello dr GEORGEBEST i lifted a carpet one and a half year ago and then i had wet dreams for more than a week i went to the doctor who gave me some medicines and told me prescribed me physiotherapy for10 days i took it for 9 days and since then i have been having this problem i took x-rays which shows disorderness in my lumbar spinal nerve yet i dont know whats the problem is it sciatica or disc herniation or nerve compression or somethin else would u determine this for me please.

@dannylamb456 Sciatica can occur at any age, although it is unusual for someone as young as you. I suggest you start by taking a look at my videos, Sciatica Exercises Part 1 and Part 2 and also visit my website shown on the videos to get my free ebook that has basic self-treatment information. In many cases, the self-treatment methods will ease symptoms considerably within a few days. Also make sure your parents are aware of the problem so they can get additional help for you if needed.
@dannylamb456 I'm not sure what you mean by the nerve is caught on a bone because that is not really something that happens except bone spurs growing into the nerve and you're too young for that to be the problem. The vast majority of the time, the problem is a disc protrusion and/or the piriformis muscle, and they are discussed on the two videos I mentioned previously. It may take a few days to a week of consisitent use of the methods on those videos and in my ebook to know if they will help.

@dannylamb456 Hi Danny, I always recommend starting with the basics - the McKenzie exercise in the position that best centralizes symptoms (as explained in my videos and ebook on my website), piriformis stretching as shown in the Sciatica Exercises Part 2 video, and applying cold packs to the low back as directed in my ebook.See how that goes over the next few days and if it's not getting better, then you may need something more advanced or perhaps professional treatment.
@DrGeorgeBest I got an MRI on my back and the DR. said the nerve was caught on the tail bone and asked if I wanted to get surgery or rehab. I have had pain in the lower region of my back and right leg for about 7 months now and had spasms in my right mid back for 3 years. My Question is what do you recommend i do while waiting a month for my next X-ray? I LOVE to do cardio/lift and want to start again because the rest has started to become counterproductive, last 30 days of a 90 day break.
@lukeakavanagh Perhaps there was some miscommunication, but I don't know how it would be possible for the nerve to be caught on the tailbone. A second opinion would probably be a good idea in any case if you are considering surgery at all. As to exercise, you can probably do some low-impact cardio such as walking, elliptical, swimming, etc., just take it easy at first and build up gradually. My website has quite a bit of self-treatment info and that will likely help at least somewhat too.

I train a fighting sport, and I have a strong lower back pain. I had it for a few weeks but it wasn't that strong. One day after the training I almost couldn't move, but it got better after about half an hour. This Tuesday I was jogging with sprint intervals. after I finished I couldn't walk normally, but this time it didn't go away. For two days I couldn't go out of my house. Now it's a litle better, I can sit down and stand up and walk

@wanjabaraba My question isn't if I'm going to be able to fight in 20 days, it's how to get better as quick as possible. I'm not missing this opportunity if I have to fight injured.
@wanjabaraba I suggest you check out my Sciatica Exercises Part 1 video and start with that exercise. You can get additional self-treatment tips by registering at my website, shown on the video. In addition to the self treatment methods from my website, I suggest you take a few days off from training to try to get the back problem sorted out. You may need to train, but if you don't let the back settle down, you won't be able to train all that well anyway, and it will delay your recovery.
I don't feel much pain now but my back get stiff some time, can you please tell me does it ever get better and can I do weight lifting again because Its my passion and my life.
@SuperMightykiller You can still lift weights, but I'd recommend avoiding any heavy lifts in which you are bent forward at the waist (such as straight leg dead lifts, unsupported bent rows, etc.), or back extensions against resistance. With a spondylolisthesis, you need to keep the abs as strong as possible to prevent more slippage of the vertebra. You'll probably always have some back soreness at least off and on but strong abs and the occasional massage and/or chiropractic will minimize it.
hello doc best i have really bad pain on my right leg and its at my hip and then i feel a really bad pain in my right calf at the right i experience this pain after i went to a party and i was dancing find until i got thing bad pain and i could berly sit and that happen maybe 2 months ago and now my right calf is still hurt super bad and i work standing up basically for 6 hours and it hurts so much and i fo feel a numbness too .. is there a way that you can please help me with this im only 20
@iheartmebabiies The first step is to try to figure out what th cause is - a bulging disc or muscle problems. I suggest you try the exercises shown on my Sciatica Exercises Part 1 and Part 2 videos. If there is disc involvement, the exercise in Part 1 will probably help. If there's no change at all with the Part 1 exercise, then the problem is probably coming from the pirformis and/or gluteus minimus muscle. See how things go with the exercises and let me know if you have further questions.
I should come see her again and she will do an MRI and refer me to a specialist. I am only 21 years old, should I be experiencing back problems at this young age? Also, should I heed my doctor's advice and wait it out for 1 more month. If the pain hasn't gone away for the last 2 months, I dont think it will go away in 1 more month. I have never had any injury or back trauma, I dont understand how I got these herniated discs.
@punjabistan01 Sciatica is related more to what wear and tear your back has had rather than how old you are. In many cases, disc problems begin from microtrauma like prolonged slouched sitting. I agree that waiting another month probably is not going to change anything. I suggest you try the exercise in my "Sciatica Exercises Part 1" video and the treatments from my free ebook on my website. If you aren't getting any better within a week or so, I'd suggest trying to get your Dr. to do the MRI.
I have been experiencing pain in my right leg for the past 2 months. The pain starts from my buttock and goes down my hamstring. The pain is accompanied by a burning sensation and numbness. My foot also experiences numbness, a burning sensation and tingling. I got my x-ray taken and the doctor said that there is pinching in my lower back discs, but she said it isn't of any immediate concern. She said that I should wait for about a month and if the pain still doesn't go away....see 2nd comment

I believe I hurt my sciatica nerve by doing an exercise where I was on a machine that hold your leg by the ankles and you do bend overs what looks like a short lectern and then you cross your arms and then do the bends (sort of like standing situps) Well one time I felt I hurt something which I think led to all this pain. Maybe a disc. My question is, at 67 will I be able to right this problem? It's been hurting for approx 3 weeks now. Thank you so much.
@octojake Most likely you can manage the problem with exercises and self-treatment. I suggest you check out my Sciatica Exercises Part 1 video and try that exercise to see if it helps. Thanks Doc for the amazing explanation. I dont think there's any body pain worse than sciatic nerve pain and mine was caused by nothing but a mattress. Pls people, get rid of your innerspring coil mattresses because they are the silent killers. These devils are destructive grills of steel layered with poor quality foam that quickly degradess causing the mattress to sink in and subjecting your muscles and nerves to pain from the springs. Give other types of mattresses a look no matter the cost.
@Espenji Thanks for your comment. While some people do fine on just about any bed, a mattress that is not providing proper support is a common source of back problems. If you go to bed feeling OK, but wake up with a lot of pain, that is an indication that your bed is at least part of your problem.

@DrGeorgeBest hello my english is bad but i will give it a try... im 22 years old,i went to see a chiropractor recently,he took some xrays of me and he told me that the L5 is pressing on my nerve(dont know how to say this correctly)when i do exercises for my abs i feel the l5 is cracking but its not painful and i dont have any back pain, but my lateral hamstring of my right leg feels like its weak and out of alignment and it doesnt function properly... how serious do you think this ?
@dzionisis If the right hamstring is a lot weaker than the left, it coud be a sign of nerve compression, which could be serious. If it is just a little weak, it's probably related to some problems in the lumbar spine joints and/or the sacroiliac joint which is not too serious if corrected before major muscle imbalances develop. I suggest trying a few visits with the chiropractor to see if he can help. If you aren't improving within a few weeks, an MRI would be a good idea to evaluate further.
@dzionisis that is excatly what i feel when i lift my leg or do circles with my anckle i feel those cracking nerves and i have the L5 problem too and it hurts too bad when i stand for like 2 hours straight or more its horrible but when i doo some streching technic it helps i little but it comes back
have a question Im 40 year old men never have any problems very active in life buf after I fall and twist my back I have two operations L4 and l5 herniated disc my accident occurd on 2/8/11 and is 6/14/11 and still have really bad pain and leg nubmnes and spasomms the Insurance send me to their own doctor and he told me I may need one more surgery and he stop the terapy my doctor who operated me the first time have no clue thecost the nubmnes my famili is safering
@california342 I'm not sure I'm answering your question, but it sounds like you probably are on worker's compensation. Every state has its own work comp rules, but most have some appeals procedure that you can use to get continued therapy paid for after the insurance company cuts you off. I suggest you contact your state worker's compensation commission and ask if there is someone who can assist you in finding out what your rghts are and how to get the treatment you need.
@DrGeorgeBest, thanks for the video, i got a good understand. as you said in the video i have pain in my lower back pain, i suddenly get it and i get it by a small cramp or sprain in the lower back. then i have to take rest, some times even if i take rest it stays for a while normally 1-3,5 days and it is horrible. i can't even move, stay as is, sleep, sit. i have it for 10 years. please suggest me. (in L4,L5 disks, doc said its nerve or muscle problem, gives me pendure injection. PLEASE HELP ME
@ramukotha It sounds like you probably have a bulging disc that periodically gets inflamed. I suggest you start using the exercise in my "Sciatica Exercises Part 1" video. When you are not in much pain, it is good to do that exercise a minute or two a few times each day. When you are in pain, doing it for a minute or so at a time up to 4 or 5 times per hour you're awake may help relieve the pain much faster. Good luck!

I have only numbness not pain in my right buttocks when i sit, my doctor diagnosed Sciatica. Two rounds of Prednisone have not helped. Is this a disk or muscle problem?
@utubeghans I can't really say what the cause is without being able to evaluate you in person. I suggest you try the exercises/tests in my YouTube Videos, Sciatica Exercises, Parts 1 and 2 to see if that helps you distinguish the cause. Ultimately, if the problem does not resolve, I'd recommend an MRI of the lumbar spine to evaluate further.

I tried deadlifting with my sciatic pain and I experienced mass pain, I instead tried kettlebell getup, again terrible pain. I cant seem to do much with my leg is so painful.
@mysticpiggyx It's best to layoff any resistance training while you're having sciatica. I recommend you focus on trying to get the sciatica under control first. Once the sciatica is settled down, you'll probably be able to return to weightlifting, but you need to allow things to heal. I was in keen pain in my leg,at last i get relief from salt METHYCOBALAMINE INJECTIONS.I took only 6-7 injections of Mehylcobalamine .This salt give strength to nerves
@bikker100 I am happy to hear that the methylcobalamin (also known as vitamin B-12) injections helped you. This treatment is useful in nerve damage caused by vitamin B-12 deficiency (pernicious anemia) and in some cases of diabetic neuropathy. Most cases of sciatica and sciatica-like symptoms are caused by other things, but when nerve pain is not resolving, a blood test to check for B-12 deficiency is definitely worth checking into, especially in elderly people, alcoholics, and diabetics.
@NiggadickJaxHD I suggest you try the stretch shown in my video, "Sciatica Exercises, Part 2" and perhaps some deep massage of the piriformis muscles. In some cases, there will be reactive spasm in the piriformis following surgery that may be associated with problems in the sacroiliac joints. If stretching and massage help but don't provide lasting relief, then I'd suggest seeing a chiropractor who uses a low-force technique like Activator or Sacro-Occipital Technique. Good luck!
@tevktoreen Just anti-inflammatories are not enough. Narcotics like hydrocodone or oxycontin are superior. Seizure meds like gabapentin are also great. Change your doc if she refuses to prescribe these.

Now I'm not quite sure if my pain is sciatica. It does not radiate to my leg and is not on the middle of my buttock, instead a little higher on the right side of the lower back. The rest of the symptoms are very similar. I've had it once before lasting months until I decided to see a chiropractor and a few weeks of visits made it go away. @yidaf18 While what you described is technically not sciatica, it sounds like it probably is due to the same underlying cause - a bulging disc. In such cases, the exercise shown in my "Sciatica Exercises, Part 1" video along with the use of cold packs will usually help. Chiropractic treatment is also usually effective, as you have experienced for yourself in the past. If you have questions, let me know.
I have had severe chronic back pain for over 6 years and have been put on extremely strong pain medication and physical therapy, both of which did nothing for it... the pain meds only masked it. I have been getting steroid injections which dont last now they want to try injecting the joints in my back rather than the epidural space. I am only 33 yrs old and I am so exhausted with living with this, I have children and can not do many of the things I should be doing with them.
@voodooma30 You did not mention what your diagnosis is.Symptoms related to a disc herniation will usually respond to exercises such as what's shown in my video "Sciatica Exercises, Part 1" (with chronic cases, frequent use - a minute or so at a time up to 3 or 4 times per hour may be needed to start getting improvement). If there is no disc problem, joint dysfunction in the lumbar and or sacroiliac joints could be the problem, which injections might help but often respond better to chiropractic.

im 24 years old and im worried my pain is coused by the spinal problem... how can i tell what it is??? i was walking to work and sneezed, on my last sneez i kinda let go... and the bottom part of my spine kinda felt like it went out... and this was after some squat excersices i may have done incorrectly...
@arnoldnavarro21 The Sciatica Exercises Part video that you commented on tells one way to get an indication of what the problem is.With the history you gave, I'd say that a disc bulge is a likely suspect, although it could also be a strain or other injury in the low back. I suggest you try the self-treatment methods from my free ebook available on the website shown at the bottom of the videos and see how it goes. If you aren't improving within a few days, I suggest you get checked by a doctor.

@sprintbass There's probably some combination of disc and joint involvement that causes your back to tighten up. Exercises that improve flexibility, such as yoga or Pilates may be helpful. Chiropractic and massage will probably help to some degree as well.

@ESOTERICALADEPT Most cases of sciatica are due to structural/mechanical problems (as described in the video), although the associated inflammatory response is somewhat related to pH. Chronic poor posture over time, improper bending and lifting, and many other mechanical factors can gradually cause the discs to bulge until things get bad enough to trigger symptoms - often suddenly without any major identifiable cause. Alkaline pH is good, but does not make you impervious to all harm or illness.

I'm 20 years old and I've been having sciatic nerve trouble for about a year now. I've been to the ER, a doctor, and a chiropractor since then, but none of them can figure out what's causing the pain, so I'm getting worried. How bad the pain is varies, and I can't seem to find a pattern. I was in a car wreck about 6 months prior to the pain, but the wreck was minor--no one was injured and I didn't experience any pain or discomfort, so I'm really not sure what's causing it. Any ideas? @AprilDaugherty You did not mention what diagnostic tests have been done. If you have not already had one, I'd suggest an MRI. If you've had an MRI that was normal, then I'd suggest an upright MRI, since 20-30% of disc protrusions (even severe ones) may not show up on an MRI done lying down. I'd also suggest that you check out my YouTube videos, "Sciatica Exercises, Part 1 and Part 2", as they show ways to help determine the cause as well as self-treat sciatica. Good luck!

@krauty86 While I acknowledge that many health problems can be the result of emotions and subconscious thoughts, I have to disagree with your assertion that "almost all cases of sciatica are caused by repressed emotions". If that were true, then physical treatments would almost never work. I have no doubt that Dr. Sarno's methods are effective for some sciatica sufferers, physical factors would appear to be involved in the majority of cases.

@krauty86 You're entitled to your opinions about the treatments you mentioned and oxygen deprivation (ischemia) does appear to be involved in most types of pain (tissue ischemia causes the release of various chemical irritants that cause pain), but you seem to be assuming that all ischemia is due to tms. If a herniated disc is pressing against a nerve as well as compressing spinal blood vessels around it, this would cause ischemic irritation, whether the person has repressed emotions or not.
Very informative video, thank you for sharing. I did not have sciatica symptoms until my first pregnancy. With each pregnancy, it intensified, and now it's chronic, even without being pregnant. What are the different types of treatments? How can one find out what is causing it? CT scans? X-rays?
@alleybow1 Treatment of course depends on the cause. Sometimes the cause can be determined by examination (for one simple method, see my video, Sciatica Exercises, Part 2), but a definitive diagnosis usually requires CT or MRI to be able to visualize the discs and/or other possible space-occupying lesions in the spine. One self-treatment that works for many people is in my Sciatica Exercises, Part 1 video. More detailed information about sciatica is available at my website shown on the video.
I went to a doctor once and he thought it might be a muscle pressing on the nerve. But alot of doctors nowadays dont want to rush into saying what someone has for fear of misdiagnosis and malpractice. No offense. But i did not go back after thar visit to follow up. @NathanielLouis Being overweight is not generally considered a direct cause of sciatica, but it can be a complicating factor in recovering from sciatica. There is another condition (called meralgia paraesthetica) that causes pain and tenderness on the outer side of the thigh (whereas sciatica usually involves the back of the thigh) that does have an association with being overweight, and is sometimes misdiagnosed as sciatica.
@NathanielLouis You did not say whether you had been evaluated by a doctor, but as discussed in the video, sciatica is usually caused by a bulging disc, but it can be caused by other things. If you have not had an MRI or CT scan, I would recommend you do so, to try to find out what the underlying cause of your sciatica is.
@NathanielLouis I'm sorry to hear that. I don't know if you've tried the McKenzie exercises or not, but if not, I suggest you check out my YouTube video, "Sciatica Exercises, Part 1". There are some cases that are beyond conservative methods of treatment. Depending on the cause of your sciatica, it may be something that surgery is the best option for having a chance of getting relief.

I 'm feeling a lot of better as I have been doing Mckenzie technique exercise of Sciatic on daily basis. that one you showed me, I found it very helpful and the symptoms has been improving, and I don't feel pains when I walking and I used to have painson back of right calf, however sometimes I do feels numbness on my toe of right feet. Can you please advise when the numbness on my toe will go? thank you
@sonnychow1 Numbness can last anywhere from a few days to several months after the major pain symptoms are gone and in some cases it does not ever go away completely. There's not a definitive treatment for numbness in this type of case, other than continuing the things that got rid of the pain. Supplementing with anti-oxidants might help some, but there's no solid research to say for sure. Laser therapy sometimes helps too, but again, it's not a sure thing.

Can you please advise me what's symptoms like if someone's nerve is damaged by the bulging disc? At the moment I still feels numbness on my toes of my right feet, however symptoms has been improving slightly. I fear that my nerve may be damaged by bulging disc. @sonnychow1 Any symptoms related to a bulging disc could technically be considered signs of nerve damage, the question is whether the damage is permanent or not. Unfortunately, the only way to know that is to wait and see. It's generally considered that whatever recovery is going to occur will occur within 2 years and it is common for it to take several months for nerve compression symptoms to resolve completely in those cases where the damage is not permanent.

@sonnychow1 The same exercise in the "Sciatica Exercises, Part 1" video can be used for prevention - a minute or so at a time, a few times daily. Also, any exercise that strengthens the "core" - the abdominal and oblique muscles - will help to stabilize and protect the lower spine, which helps prevent sciatica problems.
I have pain down my buttock, and into the hamstring, but i've had a lumbar MRI scan and everything was fine no disc damage or stenosis, i've been in pain after exercise, had physio but still in pai, any advise? Thanks
@ikeelyouall1641 It sounds like you are probably having muscle referral pain, either from the piriformis or the gluteus minimus. I suggest you watch my "Sciatica Exercises, Part 2 - Revenge of the Piriformis" video, and look up some stretches for the gluteus minimus. You might also benefit from some deep tissue massage for those muscles too. Good luck!
@DrGeorgeBest Thanks a mil for getting back to me , i'll definately watch the vids, my physio told me it was referal pain from my obturator infernus, which in my case(large glutes) is really hard to get massaged, he's done something called dry needling on it ,that gave me relief for a while but as soon as i start exercising again boom, afterwards im in bits i get , thing is i've been strechin the shit out of it, for over a year now with no real progress thanks again
@ikeelyouall1641 Usually when the muscle won't release it's because it's compensating for another problem. My guess would be that either the sacroiliac or hip joint is not moving properly and that's keeping the muscle tight. I suggest seeing a chiropractor to get that checked out and see what can be done.Not all chiros are the same though, so try to get a referral from someone who's had a good experience or find one in the phone book with a listing only (a sign that they get referral business).
I have been suffering sciatic nerve about 5 weeks, however I still pains on back of my right calf when I was walking, also sometimes I feels numbness of my toe. Can you please advise me when pains will go forever? At the moment I am doing stretching of sciatic nerve pain which make sure my hip contacted with floor and trying to feel relaxing. @sonnychow1 It can take several months to get maximum improvement in sciatica symptoms, and in a few cases, some symptoms are permanent due to nerve damage caused by the bulging disc. i undergo for X-ray and it shows that i have a mild lumbar scoliosis as the physician said.. i went for a therapy and they give me some daily routines to do so..but it's not helping i'm flexible and i think my spine is well it's something in the sciatic nerve down to my lower right limbs.. i worry because i can't stand or walk long and when i sit it's hard for me to get up...

I hurt my back 2 months ago, i have pain that comes from the sciatic nerve shoots down the right side of my leg to my knee then my ankle. i sometimes feel the pain up to my neck and around my abdomen all along my right side it gets really painful,its also hard to drive. I had an xray done recently & Dr said that there was nothing wrong. TO, me the pain is really bad it feels like there is something wrong should i request an MRI for a more detailed look would that really even make a diffrence?
@evieloveskyle Most cases of sciatica are due to bulging discs. Regular X-rays do not show the discs, only the space that they occupy, so you can't tell whether any of the discs are bulging from an X-ray. MRI or CT is necessary to see the discs themselves, and even then certain disc problems may not be visible. In a case like yours in which the symptoms have been present for several weeks, I think an MRI would be appropriate to diagnose you further and determine treatment options. Good luck!

i went to france and walked all day stood in museums all day barely slept for like 7 days danced etc...i had a knee problem a year ago but now i have pain in both knees on the tendons sometimes on the back of one knee, stiff ankles, sore calfs on the back of them, and sometimes a lower back pain and an uncomfortable numb buttocks type of feeling...is this sciatica? i know i just overdid the walking stair climbing standing, but im 22 and healthy, a runner, and i barely can walk for 20 minutes
@keepitacrime I can't rule out sciatica but it sounds more like direct problems in the hips, knees and ankles. You may have unstable arches in your feet and that's causing the knees, hip, and back to shift and move differently in compensation. The extra stress on the feet during your trip could have brought out the symptoms.I suggest trying some generic arch supports to see if they help at all. If so, the problem is likely the arches, and you may want to consider custom orthotics.
@DrGeorgeBest ahh yes i tried getting new gel type soles for my shoes but it hasnt seemed to do any better, now also that im inactive because of it, it seems to be getting worse the more i lye around?
@keepitacrime Gel soles are often primarily designed for shock absorption and don't really help much with arch support, so depending on what type of gel soles you have, they might not be enough to stabilize the arches. Inactivity can definitely make things worse because the muscles become deconditioned and the lack of movement can allow them to tighten up even more. I suggest you try some gentle stretching, and perhaps see a massage therapist and/or chiropractor to see what can be done.
@keepitacrime i had it for 3 months, and its at its worst now, not only can i not drive, i cant work or even get changed anymore, im going for walks everyday, and now even thats becoming very hard to do. i dont know what to do anymore...
....3.I'm assuming that during the incline the sciatic nerve is getting pinched in some way.It feels to me that the pain is coming from the muscle that your showing in the video.I'm feeling a muscle in the left buttock contracting and having spasms.These spasms are also occuring in my thigh and calve.I wonder if this is somethig that a chiropractor might be able to correct??
Meds just aren't working.The pain is so aweful,it's really getting to me mentally.

@Geardoodle The increase in symptoms could be due to muscular entrapment of the nerve, or it could be that the disc herniation has worsened and the nerve gets pulled up against it more with incline.It would probably be best to avoid inversion for now. I suggest you start with my Sciatica Exercises Part 1 video and cold packs as directed in the ebook available on my website. If you don't get improvement, a new MRI would be a good idea to evaluate further. Good luck!

....2.The muscles in my left buttock,thigh and calve are aching to the point that I'm not sleeping well.I've been walking with a notable limp for about 10 days now.Like I said,this is an old injury,but I've never felt it with such intensity for this length of time.I got an inversion table last week to see if that would help.My problem that I'm having with the table is that when i incline to strech myself the leg pain becomes terrible....

1.I've had an ongoing problem with l5 disc herniation and sciatic pain for about 13 years now.Over the years I learned how to keep back strains to a minimum to as not to overly agrravate the pain.However,about 2 weeks ago,I woke up with unbelievable pain in my low back,and hip,and pain going down my leg.It's the same pain that I've had for years,but it's so much more severe and lasting now......

I've had a constant stiffness and sharp pain just above my left buttock since February of 2010. Standing too long, walking too long, or even sitting too long will send jolts of pain down through my left leg down into my foot making it quite uncomfortable...Prolonged exposure to cold weather also increases pain. I know I shouldn't be as worried as I am, but I'm going to be twenty in July and I should not be feeling this old at such a young age. :(

@Specialboy1 I suggest you check out my Sciatica Exercises Part 1 and Part 2 videos and try the tests/exercises on them. Your symptoms may be related to a bulging disc, or could be a less-serious problem of muscle trigger points, possibly associate with a sacroiliac joint that is not moving properly. If neither exercise is helpful, I'd suggest getting checked by a chiropractor to see if it is a sacroiliac problem. Good luck!

I have just undergone surgery at L5 S1 I've had sciatic pain for several years as well as muscle spasms from my toes curling to cramps all over left leg, above ankle outside of my leg gets the most problems constant twitching then cramps,I also have foot drop so am unable to operate my foot.Surgery was decompression with pedical screws for spondylolthesis also fusion,bone was also removed from nerves to give them more room.I have less pain in my lower back now but still have some pain in foot.

Hi doc, please answer, I'm in so much pain! Severe pain in lower back. It pulsates all day. Severe pain in left buttock that shoots down left leg. Left buttock has lost a lot of fat and is dented/flattened/misshaped where the pain is. Sitting is painful, standing is painful, walking hurts. I have disc bulge at L3-4 & L4-5 & degenerate disc changes as per MRI. I also have a huge bulge on my lower back that shows through my clothes. What should I do? Severe pain 24hours per day. I'm only 33. Help

@nikkianika I suggest you start with the exercise in my Sciatica Exercises, Part 1 video. Try to find the position that best centralizes the symptoms (explained in the video and in the free ebook available on the website shown on the video). Use that position frequently - about a minute at a time, up to 4 or 5 times per hour you're awake. Usually that will begin to improve the symptoms within a few days to a week. Hi Dr. I have sciatica symptoms-- pain runs from buttock area down to calf--that completely goes away if I sit down, but if I stand up it gets progressively worse. From what I've read, most people seem to have pain sitting down and not when they're walking, but i'm the opposite and wonder what that could mean in regards to my sciatic condition. @heyyy19 Symptoms that improve with sitting and increase with standing are usually related to either degenerative arthritic changes in the joints at the back of the spine or occasionally dysfunction in one or both sacroiliac joints. With degenerative arthritis, extra bone, thickened ligaments, and sometimes cysts on the spinal joints can narrow the space at the back of the openings where the nerves come out and standing places the spine in slight extension which further decreases the space.

@heyyy19 ...continued from previous post - If the problem is from degenerative narrowing of the nerve openings, flexion stretching exercises (such as lying face down on an exercise ball) and abdominal core strengthening maybe helpful. Other treatments like spinal decompression and massage can also relieve pressure on the nerves, and spinal injections can temporarily relieve symptoms by decreasing pressure from inflammatory swelling.

I get pain on my lower back (where the nerves come out and also the piriformis is). sometimes it's really painful specially in the cold weather. I spoke to my GP and she said with the exercise the pain would go away yet nothing's happened.
I have a big curve on my lower back as well and when I walk about 20 minutes the pain on back doubles so I must sit down or bend myself and touch my toes 3 to 5 times and it gets better. I'm a man and only 32 years old. Could you advise me what to do please?

@kataSang Since the pain gets better after touching your toes, it's probably not a disc problem. There's probably some type of dysfunction in one or both of the sacroiliac joints that is causing inflammation and muscle spasm. Sometimes, this will go away on it's own, or with stretching, but it may to be corrected with manipulation. I suggest you try the stretch shown in my Sciatica Exercises Part 2 video and if it does not help, then consult with a chiropractor or osteopath.

Injections may provide temporary relief by reducing inflammatory swelling, but will not correct the disc protrusion. The temporary reduction in symptoms may allow you to do things that can help the disc protrusion (such as the exercise on my video), so the injection may be worth considering. Whether or not you do the injection, I suggest you ask your doctor to refer you to a PT, chiropractor, or physical medicine doctor to help with pain and then for help with rehab to prevent a recurrence.

I did not want a temporary fix, but I have not slept for a week, and I can't go on like this. I am not sure why they are so opposed to surgery unless it is not very successful. If I knew surgery (microdiscectomy) would be a permanent solution I would do that in a heart beat. I don't want this to keep coming back.

@MultiNelson69 Surgery in general doesn't have the best long-term results, especially the more aggressive types which often create problems with scar tissue development later on. Minimally invasive surgeries are usually a good option when non-surgical methods fail, but you can't really think of surgery as a "permanent" solution. You still need to be careful with your back and do preventive exercises to minimize the risk of recurrence - either at the same disc or at an adjacent one.

HELP! I have 5 MM left paracentral disc protrusion impinging on the traversing left S1 nerve root. My doc wants to send me to a radiologist for injectio. Is this the best course of action? This is probably the beginning of many problems I will have in the future. I would like a doc who will work with me on preventive measures like exercise, diet, ...whatever. I am afraid the radiologist will just do injections -not over all care. Who would be the best type of doc to see in your opinion?

I am experiencing sciatica symptoms. It feels better if I walk as opposed to resting. It always gets worse in the afternoon and I go progressively down hill from there. I had an MRI on Friday and am waiting for the results. My doctor gave me pain pills but I try to save them for the night so I can sleep. I tried swimming today and it HURT A LOT! Why does it get worse in the evening? It doesn't make any difference if I am active or rest all day,,,it always gets worse in the evening.

@MultiNelson69 In many cases, the underlying issue with increased pain late in the day is inflammatory swelling. Many people will have an increase in inflammation (regardless of activity) late in the day because their levels of the hormone cortisol will often drop at this time (this is often seen in chronic stress as the adrenal glands get overworked). I suggest you check out my Sciatica Exercises Part 1 Video and/or visit sciaticatreatmentathome . com for self-treatment tips.

Hi, I am a volleyball player and I once stretched too much behind when I was spiking. The pain has lasted for months, even now. I can't sleep well with this pain. The pain goes from my left lower back to my leg. When the pain occurs my left leg would turn all wobbly and it goes away after approximately 5-7 minutes. I wonder if it's sciatica as the symptoms are really close. Is there any way to cure it and what type of sciatica do I have?

@Leeteukluv It could be disc-related sciatica, but since the symptoms go away after a few minutes, I think it could be a problem in the sacroiliac joint. I suggest you try the exercise shown in my "Sciatica Exercises Part 1" video. If that helps, it's probably a disc and repeated use of the exercise will probably get rid of it over time. If that doesn't help, it's probably sacroiliac and I'd suggest you see a chiropractor or osteopath to get it checked and treated.

@orhun20 Your symptoms are most likely related to a bulging disc. I suggest you try the exercise shown in my "Sciatica Exercises Part 1" video on a frequent basis for a few days to see if it helps. I also recommend you use cold packs as directed in my ebook (available on the website shown on the video). If you don't get any improvement within a week or so, I'd recommend seeing a doctor and getting an MRI to evaluate the situation and find out what the treatment options are. Good luck!

@dannylamb456 Sciatica can occur at any age, although it is unusual for someone as young as you. I suggest you start by taking a look at my videos, Sciatica Exercises Part 1 and Part 2 and also visit my website shown on the videos to get my free ebook that has basic self-treatment information. In many cases, the self-treatment methods will ease symptoms considerably within a few days. Also make sure your parents are aware of the problem so they can get additional help for you if needed.

@dannylamb456 I'm not sure what you mean by the nerve is caught on a bone because that is not really something that happens except bone spurs growing into the nerve and you're too young for that to be the problem. The vast majority of the time, the problem is a disc protrusion and/or the piriformis muscle, and they are discussed on the two videos I mentioned previously. It may take a few days to a week of consisitent use of the methods on those videos and in my ebook to know if they will help.

@DrGeorgeBest I got an MRI on my back and the DR. said the nerve was caught on the tail bone and asked if I wanted to get surgery or rehab. I have had pain in the lower region of my back and right leg for about 7 months now and had spasms in my right mid back for 3 years. My Question is what do you recommend i do while waiting a month for my next X-ray? I LOVE to do cardio/lift and want to start again because the rest has started to become counterproductive, last 30 days of a 90 day break.

@lukeakavanagh Perhaps there was some miscommunication, but I don't know how it would be possible for the nerve to be caught on the tailbone. A second opinion would probably be a good idea in any case if you are considering surgery at all. As to exercise, you can probably do some low-impact cardio such as walking, elliptical, swimming, etc., just take it easy at first and build up gradually. My website has quite a bit of self-treatment info and that will likely help at least somewhat too.

I train a fighting sport, and I have a strong lower back pain. I had it for a few weeks but it wasn't that strong. One day after the training I almost couldn't move, but it got better after about half an hour. This Tuesday I was jogging with sprint intervals. after I finished I couldn't walk normally, but this time it didn't go away. For two days I couldn't go out of my house. Now it's a litle better, I can sit down and stand up and walk

@wanjabaraba I suggest you check out my Sciatica Exercises Part 1 video and start with that exercise. You can get additional self-treatment tips by registering at my website, shown on the video. In addition to the self treatment methods from my website, I suggest you take a few days off from training to try to get the back problem sorted out. You may need to train, but if you don't let the back settle down, you won't be able to train all that well anyway, and it will delay your recovery.

@SuperMightykiller You can still lift weights, but I'd recommend avoiding any heavy lifts in which you are bent forward at the waist (such as straight leg dead lifts, unsupported bent rows, etc.), or back extensions against resistance. With a spondylolisthesis, you need to keep the abs as strong as possible to prevent more slippage of the vertebra. You'll probably always have some back soreness at least off and on but strong abs and the occasional massage and/or chiropractic will minimize it.

@iheartmebabiies The first step is to try to figure out what th cause is - a bulging disc or muscle problems. I suggest you try the exercises shown on my Sciatica Exercises Part 1 and Part 2 videos. If there is disc involvement, the exercise in Part 1 will probably help. If there's no change at all with the Part 1 exercise, then the problem is probably coming from the pirformis and/or gluteus minimus muscle. See how things go with the exercises and let me know if you have further questions.

I should come see her again and she will do an MRI and refer me to a specialist. I am only 21 years old, should I be experiencing back problems at this young age? Also, should I heed my doctor's advice and wait it out for 1 more month. If the pain hasn't gone away for the last 2 months, I dont think it will go away in 1 more month. I have never had any injury or back trauma, I dont understand how I got these herniated discs.

@punjabistan01 Sciatica is related more to what wear and tear your back has had rather than how old you are. In many cases, disc problems begin from microtrauma like prolonged slouched sitting. I agree that waiting another month probably is not going to change anything. I suggest you try the exercise in my "Sciatica Exercises Part 1" video and the treatments from my free ebook on my website. If you aren't getting any better within a week or so, I'd suggest trying to get your Dr. to do the MRI.

I have been experiencing pain in my right leg for the past 2 months. The pain starts from my buttock and goes down my hamstring. The pain is accompanied by a burning sensation and numbness. My foot also experiences numbness, a burning sensation and tingling. I got my x-ray taken and the doctor said that there is pinching in my lower back discs, but she said it isn't of any immediate concern. She said that I should wait for about a month and if the pain still doesn't go away....see 2nd comment

I believe I hurt my sciatica nerve by doing an exercise where I was on a machine that hold your leg by the ankles and you do bend overs what looks like a short lectern and then you cross your arms and then do the bends (sort of like standing situps) Well one time I felt I hurt something which I think led to all this pain. Maybe a disc. My question is, at 67 will I be able to right this problem? It's been hurting for approx 3 weeks now. Thank you so much.

Thanks Doc for the amazing explanation. I dont think there's any body pain worse than sciatic nerve pain and mine was caused by nothing but a mattress. Pls people, get rid of your innerspring coil mattresses because they are the silent killers. These devils are destructive grills of steel layered with poor quality foam that quickly degradess causing the mattress to sink in and subjecting your muscles and nerves to pain from the springs. Give other types of mattresses a look no matter the cost.

@Espenji Thanks for your comment. While some people do fine on just about any bed, a mattress that is not providing proper support is a common source of back problems. If you go to bed feeling OK, but wake up with a lot of pain, that is an indication that your bed is at least part of your problem.

@dzionisis If the right hamstring is a lot weaker than the left, it coud be a sign of nerve compression, which could be serious. If it is just a little weak, it's probably related to some problems in the lumbar spine joints and/or the sacroiliac joint which is not too serious if corrected before major muscle imbalances develop. I suggest trying a few visits with the chiropractor to see if he can help. If you aren't improving within a few weeks, an MRI would be a good idea to evaluate further.

@california342 I'm not sure I'm answering your question, but it sounds like you probably are on worker's compensation. Every state has its own work comp rules, but most have some appeals procedure that you can use to get continued therapy paid for after the insurance company cuts you off. I suggest you contact your state worker's compensation commission and ask if there is someone who can assist you in finding out what your rghts are and how to get the treatment you need.

@ramukotha It sounds like you probably have a bulging disc that periodically gets inflamed. I suggest you start using the exercise in my "Sciatica Exercises Part 1" video. When you are not in much pain, it is good to do that exercise a minute or two a few times each day. When you are in pain, doing it for a minute or so at a time up to 4 or 5 times per hour you're awake may help relieve the pain much faster. Good luck!

Ultimately, if the problem does not resolve, I'd recommend an MRI of the lumbar spine to evaluate further.

@mysticpiggyx It's best to layoff any resistance training while you're having sciatica. I recommend you focus on trying to get the sciatica under control first. Once the sciatica is settled down, you'll probably be able to return to weightlifting, but you need to allow things to heal. @bikker100 I am happy to hear that the methylcobalamin (also known as vitamin B-12) injections helped you. This treatment is useful in nerve damage caused by vitamin B-12 deficiency (pernicious anemia) and in some cases of diabetic neuropathy. Most cases of sciatica and sciatica-like symptoms are caused by other things, but when nerve pain is not resolving, a blood test to check for B-12 deficiency is definitely worth checking into, especially in elderly people, alcoholics, and diabetics.

@NiggadickJaxHD I suggest you try the stretch shown in my video, "Sciatica Exercises, Part 2" and perhaps some deep massage of the piriformis muscles. In some cases, there will be reactive spasm in the piriformis following surgery that may be associated with problems in the sacroiliac joints. If stretching and massage help but don't provide lasting relief, then I'd suggest seeing a chiropractor who uses a low-force technique like Activator or Sacro-Occipital Technique. Good luck!

Now I'm not quite sure if my pain is sciatica. It does not radiate to my leg and is not on the middle of my buttock, instead a little higher on the right side of the lower back. The rest of the symptoms are very similar. I've had it once before lasting months until I decided to see a chiropractor and a few weeks of visits made it go away. In such cases, the exercise shown in my "Sciatica Exercises, Part 1" video along with the use of cold packs will usually help. I have had severe chronic back pain for over 6 years and have been put on extremely strong pain medication and physical therapy, both of which did nothing for it... the pain meds only masked it. I have been getting steroid injections which dont last now they want to try injecting the joints in my back rather than the epidural space. I am only 33 yrs old and I am so exhausted with living with this, I have children and can not do many of the things I should be doing with them.

@voodooma30 You did not mention what your diagnosis is.Symptoms related to a disc herniation will usually respond to exercises such as what's shown in my video "Sciatica Exercises, Part 1" (with chronic cases, frequent use - a minute or so at a time up to 3 or 4 times per hour may be needed to start getting improvement). If there is no disc problem, joint dysfunction in the lumbar and or sacroiliac joints could be the problem, which injections might help but often respond better to chiropractic.

I have had severe chronic back pain for over 6 years and have been put on extremely strong pain medication and physical therapy, both of which did nothing for it... the pain meds only masked it. I have been getting steroid injections which dont last now they want to try injecting the joints in my back rather than the epidural space. I am only 33 yrs old and I am so exhausted with living with this, I have children and can not do many of the things I should be doing with them.

im 24 years old and im worried my pain is coused by the spinal problem... how can i tell what it is??? i was walking to work and sneezed, on my last sneez i kinda let go... and the bottom part of my spine kinda felt like it went out... and this was after some squat excersices i may have done incorrectly...

@arnoldnavarro21 The Sciatica Exercises Part video that you commented on tells one way to get an indication of what the problem is.With the history you gave, I'd say that a disc bulge is a likely suspect, although it could also be a strain or other injury in the low back. I suggest you try the self-treatment methods from my free ebook available on the website shown at the bottom of the videos and see how it goes. If you aren't improving within a few days, I suggest you get checked by a doctor.

my back has always botherd me...i can prop myself up in a chair and my lower back pops and all the sudden i feel this rush of blood and relaxation there..i sepent many years as a dancer and a bicycle racer so ive always thought that i have some thing bothering the nerve

@sprintbass There's probably some combination of disc and joint involvement that causes your back to tighten up. Exercises that improve flexibility, such as yoga or Pilates may be helpful. Chiropractic and massage will probably help to some degree as well.

@ESOTERICALADEPT Most cases of sciatica are due to structural/mechanical problems (as described in the video), although the associated inflammatory response is somewhat related to pH. Chronic poor posture over time, improper bending and lifting, and many other mechanical factors can gradually cause the discs to bulge until things get bad enough to trigger symptoms - often suddenly without any major identifiable cause. Alkaline pH is good, but does not make you impervious to all harm or illness.

I'm 20 years old and I've been having sciatic nerve trouble for about a year now. I've been to the ER, a doctor, and a chiropractor since then, but none of them can figure out what's causing the pain, so I'm getting worried. How bad the pain is varies, and I can't seem to find a pattern. I was in a car wreck about 6 months prior to the pain, but the wreck was minor--no one was injured and I didn't experience any pain or discomfort, so I'm really not sure what's causing it. Any ideas? Thank you!

@AprilDaugherty You did not mention what diagnostic tests have been done. If you have not already had one, I'd suggest an MRI. If you've had an MRI that was normal, then I'd suggest an upright MRI, since 20-30% of disc protrusions (even severe ones) may not show up on an MRI done lying down. I'd also suggest that you check out my YouTube videos, "Sciatica Exercises, Part 1 and Part 2", as they show ways to help determine the cause as well as self-treat sciatica. Good luck!

@krauty86 While I acknowledge that many health problems can be the result of emotions and subconscious thoughts, I have to disagree with your assertion that "almost all cases of sciatica are caused by repressed emotions". If that were true, then physical treatments would almost never work. I have no doubt that Dr. Sarno's methods are effective for some sciatica sufferers, physical factors would appear to be involved in the majority of cases.

@DrGeorgeBest many treatments for pain such as chiropractics are simply placebo and ware off in a few days. stretching, pt, and massage are great for temporarily bringing blood to the affected area, but again that wares off too. science has also shown that continued compression of nerves will stop signaling all together, so why are people in such agonizing pain. oxygen deprivation appears to be involved in the majority of cases in my opinion. tmswiki.wetpaint.com tmshelp.com

@krauty86 You're entitled to your opinions about the treatments you mentioned and oxygen deprivation (ischemia) does appear to be involved in most types of pain (tissue ischemia causes the release of various chemical irritants that cause pain), but you seem to be assuming that all ischemia is due to tms. If a herniated disc is pressing against a nerve as well as compressing spinal blood vessels around it, this would cause ischemic irritation, whether the person has repressed emotions or not.

Very informative video, thank you for sharing. I did not have sciatica symptoms until my first pregnancy. With each pregnancy, it intensified, and now it's chronic, even without being pregnant. What are the different types of treatments? How can one find out what is causing it? CT scans? X-rays?

@alleybow1 Treatment of course depends on the cause. Sometimes the cause can be determined by examination (for one simple method, see my video, Sciatica Exercises, Part 2), but a definitive diagnosis usually requires CT or MRI to be able to visualize the discs and/or other possible space-occupying lesions in the spine. One self-treatment that works for many people is in my Sciatica Exercises, Part 1 video. More detailed information about sciatica is available at my website shown on the video.

I went to a doctor once and he thought it might be a muscle pressing on the nerve. But alot of doctors nowadays dont want to rush into saying what someone has for fear of misdiagnosis and malpractice. No offense. But i did not go back after thar visit to follow up. @NathanielLouis Being overweight is not generally considered a direct cause of sciatica, but it can be a complicating factor in recovering from sciatica. There is another condition (called meralgia paraesthetica) that causes pain and tenderness on the outer side of the thigh (whereas sciatica usually involves the back of the thigh) that does have an association with being overweight, and is sometimes misdiagnosed as sciatica.

@NathanielLouis You did not say whether you had been evaluated by a doctor, but as discussed in the video, sciatica is usually caused by a bulging disc, but it can be caused by other things. If you have not had an MRI or CT scan, I would recommend you do so, to try to find out what the underlying cause of your sciatica is.

There are some cases that are beyond conservative methods of treatment. Depending on the cause of your sciatica, it may be something that surgery is the best option for having a chance of getting relief.

I 'm feeling a lot of better as I have been doing Mckenzie technique exercise of Sciatic on daily basis. that one you showed me, I found it very helpful and the symptoms has been improving, and I don't feel pains when I walking and I used to have painson back of right calf, however sometimes I do feels numbness on my toe of right feet. Can you please advise when the numbness on my toe will go? thank you

@sonnychow1 Numbness can last anywhere from a few days to several months after the major pain symptoms are gone and in some cases it does not ever go away completely. There's not a definitive treatment for numbness in this type of case, other than continuing the things that got rid of the pain. Supplementing with anti-oxidants might help some, but there's no solid research to say for sure. Laser therapy sometimes helps too, but again, it's not a sure thing.

Can you please advise me what's symptoms like if someone's nerve is damaged by the bulging disc? At the moment I still feels numbness on my toes of my right feet, however symptoms has been improving slightly. I fear that my nerve may be damaged by bulging disc. @sonnychow1 Any symptoms related to a bulging disc could technically be considered signs of nerve damage, the question is whether the damage is permanent or not. Unfortunately, the only way to know that is to wait and see. It's generally considered that whatever recovery is going to occur will occur within 2 years and it is common for it to take several months for nerve compression symptoms to resolve completely in those cases where the damage is not permanent.

@sonnychow1 The same exercise in the "Sciatica Exercises, Part 1" video can be used for prevention - a minute or so at a time, a few times daily. Also, any exercise that strengthens the "core" - the abdominal and oblique muscles - will help to stabilize and protect the lower spine, which helps prevent sciatica problems.

@ikeelyouall1641 It sounds like you are probably having muscle referral pain, either from the piriformis or the gluteus minimus. I suggest you watch my "Sciatica Exercises, Part 2 - Revenge of the Piriformis" video, and look up some stretches for the gluteus minimus. You might also benefit from some deep tissue massage for those muscles too. Good luck!

@DrGeorgeBest Thanks a mil for getting back to me , i'll definately watch the vids, my physio told me it was referal pain from my obturator infernus, which in my case(large glutes) is really hard to get massaged, he's done something called dry needling on it ,that gave me relief for a while but as soon as i start exercising again boom, afterwards im in bits i get , thing is i've been strechin the shit out of it, for over a year now with no real progress thanks again

@ikeelyouall1641 Usually when the muscle won't release it's because it's compensating for another problem. My guess would be that either the sacroiliac or hip joint is not moving properly and that's keeping the muscle tight. I suggest seeing a chiropractor to get that checked out and see what can be done.Not all chiros are the same though, so try to get a referral from someone who's had a good experience or find one in the phone book with a listing only (a sign that they get referral business).

I have been suffering sciatic nerve about 5 weeks, however I still pains on back of my right calf when I was walking, also sometimes I feels numbness of my toe. Can you please advise me when pains will go forever? At the moment I am doing stretching of sciatic nerve pain which make sure my hip contacted with floor and trying to feel relaxing. @sonnychow1 It can take several months to get maximum improvement in sciatica symptoms, and in a few cases, some symptoms are permanent due to nerve damage caused by the bulging disc. i undergo for X-ray and it shows that i have a mild lumbar scoliosis as the physician said.. i went for a therapy and they give me some daily routines to do so..but it's not helping i'm flexible and i think my spine is well it's something in the sciatic nerve down to my lower right limbs.. i worry because i can't stand or walk long and when i sit it's hard for me to get up...
i need a couple of advice... please help!!

@samia0022 I suggest you start by trying the exercise shown in my YouTube video, "Sciatica Exercises Part 1". I also suggest you try using cold packs on the low back as per the instructions in my free ebook which you can get from my website (shown on the video). If you don't get any relief within a week or two, my recommendation is to get an MRI to further assess the situation (I don't think the scoliosis is the cause). Good luck!

I hurt my back 2 months ago, i have pain that comes from the sciatic nerve shoots down the right side of my leg to my knee then my ankle. i sometimes feel the pain up to my neck and around my abdomen all along my right side it gets really painful,its also hard to drive. I had an xray done recently & Dr said that there was nothing wrong. TO, me the pain is really bad it feels like there is something wrong should i request an MRI for a more detailed look would that really even make a diffrence?

@evieloveskyle Most cases of sciatica are due to bulging discs. Regular X-rays do not show the discs, only the space that they occupy, so you can't tell whether any of the discs are bulging from an X-ray. MRI or CT is necessary to see the discs themselves, and even then certain disc problems may not be visible. In a case like yours in which the symptoms have been present for several weeks, I think an MRI would be appropriate to diagnose you further and determine treatment options. Good luck!

i went to france and walked all day stood in museums all day barely slept for like 7 days danced etc...i had a knee problem a year ago but now i have pain in both knees on the tendons sometimes on the back of one knee, stiff ankles, sore calfs on the back of them, and sometimes a lower back pain and an uncomfortable numb buttocks type of feeling...is this sciatica? i know i just overdid the walking stair climbing standing, but im 22 and healthy, a runner, and i barely can walk for 20 minutes

@keepitacrime I can't rule out sciatica but it sounds more like direct problems in the hips, knees and ankles. You may have unstable arches in your feet and that's causing the knees, hip, and back to shift and move differently in compensation. The extra stress on the feet during your trip could have brought out the symptoms.I suggest trying some generic arch supports to see if they help at all. If so, the problem is likely the arches, and you may want to consider custom orthotics.

@keepitacrime Gel soles are often primarily designed for shock absorption and don't really help much with arch support, so depending on what type of gel soles you have, they might not be enough to stabilize the arches. Inactivity can definitely make things worse because the muscles become deconditioned and the lack of movement can allow them to tighten up even more. I suggest you try some gentle stretching, and perhaps see a massage therapist and/or chiropractor to see what can be done.
