For the text, that's just the Trapcode Shine effect in Adobe After Effects. I used Premiere Pro to composite the rest of the video.

At about 00:13, there is a clip with engravings running up the side of a sword. What movie/TV show is that from, or did you do it yourself?

It's from the unrated DVD version of Blade Trinity. There's a bit after Blade meets the Nightstalkers where Hannibal rants for like five minutes about Drake and there's a few cut shots of him in action. That clip is from one of those shots.
