A lot of the old rounds have malfunctioning safties, and have gone "bad". Meaning, as long as the rocket ignites it can blow up(10 meters i think). but you are sorta right. In this case the RPG had a bad booster charge, and the propellent didnt burn correctly, so no rocket ignition and no detonator activation. they would all be ok putting a new booster on there and firing again.

btw: some rounds are so dangerous that dropping them could cause explosion, but those are rare.

Also, the fuze is said to be fall proof up to 3 meter... so maybe, maybe not.... guess it depends on its age and quality.

it's a hard foam at the end, right behind the aluminum turbine, that its in the narrowest part of the launcher. it plugs the back of the expansion tube to create overpressure in which the rocket is boosted out while the foam brakes up and is forced out the blast cone (along with unburned cardboard and etc) at the same time. when u dont have that plug, booster charge cant build enough pressure to shoot the rocket out like in this vid. (there maybe other reasons for the failure though)
