Iommi actually starts the riff on the seventh fret of the E string, then moves to the 5th fret of the A string, then seventh, then goes to the 5th & 4th frets on the D string, then back to the A string at the 5th then seventh frets.

All of it is root/fifth power chords, and the bend at the start is bent up behind the nut on the open E string, while playing the E on the second fret of the D string at the same time to create dissonance.
