According to the ITTF handbook, for the playing arm, wrist up is the racket. That includes your hand.

What if the ball unintentionally hits the hand of a player's racket hand? Is that a fault or will the point be replayed?

but it is perfectly legal to intentionally hit the ball with your hand while holding the bat! Don't know why you would want to do that, but it is perfectly legal!

What if you hit the ball with the hand that is not holding the bat but you are still holding a bat in one of your hands? Is that legal?

@schwayse If it was a serve, it's a let and the point is played again. If it's during a rally, hitting the net is legal and whoever wins the point after it hits the net wins.

@schwayse Wow yeah that's strange, I totally read your comment to say 'ball'. My eyes deceived me! In fact, it's a good question and I'd also like to know that answer. My guess is that you'd lose the point based on the convention of most other racket sports but I've never read anything official.
