This guy did NOT die.. He simply opened a dry switch and it let out the fire, but he wasn't even burned. It's ridiculous the number of people who spew out "HE DIED" like they have a fucking clue.
I saw this video in school when I was an apprentice. Supposedly the breaker was not aligned properly and as he racked it in he crossed the phases. They also said he was not an electrician, he was a building engineer. Also he absolutely died, ppe probably can't help you in an incident this severe. Ibew for life
@nickbz83 I rack these types of brakers at work (4.1 and 4.8kv.) They are so well built, I could not see how ours could be installed incorrectly. I am thinking the breaker was closed (big mistake, check semaphore), , and he racked it into a solid fault, and the breaker failed in his face. After seeing this video, I ALWAYS stand to the side of the cabinet, not in front, with full PPE, so only my arm is exposed.
Looks like he was racking a HV breaker in or out of service. The breaker should have been tripped open mechanically. If it wasn't that could cause the arc flash, or he could have closed into a fault. Note the immediate burn through at the rear of the switchgear. Probably, no amount of PPE could save you from this force. PPE does nothing for concussion. And a continuity tester woul not do anything for you either.
some breakers have to be pumped for them to be turned on. Either there was a tool or piece of metal between the phases, or the phases were landed wrong, or a wire got skinned and touched metal or bus bar, could even be improperly torqued bolts. Without the real report, it's hard to say. My guess is the tool, or crossed phases. His gear may have been proper at the time. Today OSHA requires more protective gear on any live parts, ex. 480V and up full arc flash suit.

So would I. It looks to me tike he was cranking a breaker into that swithgear with live busses.
Not at all an uncommon practice, but obviously not worth the risk.
I don't kno what happened here, but I've known people who escaped from accidents like this with minmal injuries. I hope they were part of that group.
At least they were wearing some appropriate PPE.
