It has nothing to do with their throats...they are shaping their larynx (vocal chords) through controlled breathing techniques which changes pressure in the larynx which cause their vocal chords to vibrate at several frequencies simultaneously (the harmonic tones).
No, these types of singing (whether it's xoomei, gyuto, death growl, or just the generic raspy rock grit sound) if performed correctly are harmless to the vocal folds. The only problem is when someone who doesn't know the technique tries to mimmick it. They end up screaming or grunting...and of course it starts to hurt. But these guys (and Tuvan herdsmen, rock singers or death metal vocalists for that matter) SING, they DO NOT grunt/scream/whatever.
Done properly, it shouldnt hurt your throat. It only hurts if you're using the wrong part of your voice. This goes for all forms of singing, and voice usage in general.
