Is it because Nick's using an "unreliable narrator" here & people aren't quite sure of the song's intentions? They think "am I supposed to think this addict is cool?" The irony isn't as apparent as in, say, a Randy Newman song, partly because drug addiction isn't a black-and-white matter.
