He had gained a lot of weight by the time this video came out in 1990, which probably means this was one of his brief clean periods where he was starting to somewhat recover. By this point though, his voice was halfway shot and no where near as strong as it was in the 80's. Still a great song though and 100x better than stuff out these days.
