In the show of Evangelion, it's said that the Evas are actually humans, so that would explain the organs. As for the pain, in the show, they wear those head things and suits to connect their nerves to the Evas. Because of the Lance of Longius, Asuka was unfortantly able to not only feel the pain the Eva felt, but she also was able to recieve the same exact damages done to her body. I totally suggest this movie to anyone. This show rocks!!
@helenw713 In theory, the damage to the AT field done by the LoL clones did the damage, IE when they come back from the LCL sea her wounds are still there.

Her synchronization was actually around 300%. So for everything the EVA felt . . .
