Simple version: There was a character in this anime/game who was awsome, and a guy typed that he was 'GAR' for him, and meant to type 'gay'. Since then, anyone awsome enough has been given the title 'GAR', like TTGL's Kamina.
And BDO isn't in probably due to the fact that squaresoft/square-enix owns it and the game it is in.
