From the forklift operator damaging welding gas cylinders to a cutting torch man who appears to have less sense than a seven year old boy, it is virtually beyond belief. I saw this video a few years back in an OHS class, apparently nobody died, the guys who was cutting managed to jump out of the cherry picker just in time.
A lot of people got their arses kicked big time over this

I'm not so sure that someone getting hurt or even dying was unavoidable. In my experience, stupidity or carelessness (take your pick) will catch up with you sooner or later because it's a habit.

OMFG, how did they not see that it would collapse that way,ive got only 6 months experience in demolition but as soon as i saw him cutting the beam in this video i knew it would fall, the beam was stopping the legs spreading, how did they not see it was doing that job?
