important thought or action, if being with your friends or loved ones be with them, the mind will travel to some other place we cannot control that but we can remind our mind to come back to the present important thing that is to be with family and friend.
i can't talk about medicine but my view is that which ever state of mind you are in is fine the only thing which is important is how we use that stat of mind. concentrate on one thought, or one feeling or one object
now in detail, before you sleep think of one thing that you want to dream about, it is not important if you dream of it or not or if you remember the dream, think of that one thing every night, this will help you in concerting your thought to one object. and slowly after a long time before going to sleep don't think at all, but this has to be done after few months of sleeping with one thought.

now about sleep, i used to put my self to sleep by thinking of so many thing that it was a cluttered sleep with dreams which had no relation to each other. i thought and thought until i slept but now i understand a pure sleep is an empty and a deep sleep much like death, but in this case it is a healthy sleep since when we sleep with a clear mind, we wake up with a clear mind.
this is hard to do, but the first step is to have one thought instead of many. eventually no thoughts.
you are not the only one who cant differentiate between the real world and the imaginary one, no human can and i mean it no one can. the mind will have to be completely empty of any thought for us to realize reality or the truth, the real truth is one which has no belief system in it and one which is common knowledge understood by every one.
and till now no such reality exists. only science can give such a truth but it will be a long time in the future when we know it.
