"Regina" is not a middle name; it is Latin for "queen", whereas "Rex" is Latin for "king" - thus the Queen's eldest son Charles will become "Charles Rex" at his Accession. All British Sovereigns sign [Christian name] R, as in for example "Elizabeth R".

@fallingblack - this is a Technicolor film of the Coronation which was shown in most UK cinemas in 1953. The B&W footage one usually sees is from the BBC's coverage. Actually the BBC stuff in the Abbey is better - the producer ignored the strict instructions of the flunkeys in charge and got some pretty good close ups of the Queen in procession.
