I've never seen a Caldwell Luc performed like that before. I'm wondering, how did you keep the bone fragment in place--i.e. how did you prevent it from falling into backward into the sinus?

Pay attention on the angle of the saw during procedure. That is the main reason why this osteoplastic approach can be done.
