identifing Phyllobates leucomelas (Means et al)
Look up phyllobates leucomelas on dendrodata and you should be able to find the paper. Dendrodata(.)COM, for some reason you can't post links on comments).
Azureus is a morph on D.Tinctorius. Tinctorius and Leucomelas are both in the genus Dendrobates. The taxon was reviewed in 2006 (grant et al)
Phyllobates only has 5 species, including the most toxic of all poison frogs, P.Terrabilis.

There is no such thing as an azureus tinctorius and leucomelas was placed in the Phyllobates genus in 2004.
