It's just C, F, and G. On the last go 'round I added A and Dm just to make it a bit different. I used a capo to pitch it a bit higher.

I think it is called G 9 over 6. The thumb goes on the third fret, over the E and A strings, creating the notes of G and C. The ring finger goes on the third fret over the little E and the B strings, creating the notes of G and D. The index finger fits on the second fret on the two middle strings. It's a pretty cool way to end a song in C. To end a song in D, just move it up two frets.
