I actually had the same problem today and got it fixed fast :P here's a little guide for those who haven't found the solution yet.
Just remember these 2 steps:
1) Settings > General > Date and Time > (Here, switch off "Set Automatically" and manually select your hometown). Then manually set the date and time.
Apparently the SIM and carrier wont work if date and time aren't set well, make sure date and time are the current ones
2) Settings > Phone > Show My Caller ID > Switch ON
i also facing same prob. sometime when I reset verything like waht u did it works. but mostly not. so frustrated. even i go to network provider centre they doing the same. huh..
even the problem can be solved using those was above, i believe those ways are not the standard for iphone (because only mine got that problem), no guidance or faq in iphone manual from apple website how to solve if we have that problem.
so i decide to come to operator where I bought this phone, then change it with the new one.
instead of doing those above steps again and again, bring your iphone to the store and asking them to change with the new one will solve that problem permanently.
what i did r turn on/off airplane mode and call waiting, then open hide number menu, the iphone will works properly. But that's only a temporary solution, whenever you turn off your iphone, the problem will be back!!, you should do those above steps to fix that

i have same problem. but not with my friends.
