I am having the _exact_ same problem. For some strange reason the upper piston fires to soon, even though it is not powered, and the side pistons are unable to push it. My pistons are also facing f=3. I am guessing it has something to do with the repeater powering the wrong piston. Moving everthing one block solved. You're not the only one who has seen this bug in 1.8.1...
