@psycosanches589 It's a wound of some sort, at the end of the last part, he let Jonas and Emma escape, and then held the door down so the two people inside couldn't chase after them, so my guess is that they attacked him, and he narrowly escaped, the echoing is because they're in an abandoned hospital, where there is literally no sound other then their own voices, so there would be an echo.
