Well, Death Guild, who runs Thunderdome at Burning Man, does make serious efforts to keep people from getting hurt badly. You have to sign a waiver before going in and they have a medic on standby.

The point I'm trying to make is that unlike what is depicted in the film, this is done for fun and enjoyment. Getting stabbed with a glaive or beaten with a giant hammer is neither fun nor enjoyable.
