Gaz was killed by Zakhaev back in CoD4. He got killed by a bullet to his head. The reason why Ghost has called Soap by that nickname instead of referring to him as Captain MacTarvish could be that Ghost might've known Soap and have worked along side him during the time before MacTarvish gets promoted to being a Captain (possibly at the time frame before/during/after the CoD4). It could also be that Ghost might have called Soap as Captain, but then MacTarvish preferred being called "Soap".

Look at the video, they only dump gasoline on Roach for some reason and shepherd throws his cigar at u and then leaves...its possible ghost survived the gunshot wound and crawled to safety after. However Price assumes he's dead but thats only his assumption, one can never know. Price has appeared to die twice now at the end of both games only to get back up.
