I'm not sure about the other "unsent" letters, but I have a theory about this one. I think Jeremiah did receive it, but I think Carno's first evil act after examining the bok was killing poor Jeremiah, so he took the incriminating letter back with him.

The only element I ever noticed tying the first and second Phantasmagoria together was a cameo of Harriet in the sequel. Both games do drag rather badly which is a shame considering they might have helped Sierra stay afloat if they did better.
She's in the bit where he hallucinates that he's back in the asylum - she's called the Rat Woman. And I think that the second one is better than this one.
