Only the offencive skill with the highest impact on combat level is counted towards it. Having magic or ranged at 99 adds about 47 to your combat level, however, having 99 in both attack and strength adds 64 to your combat level. Because you trained both attack and strength to 99 you are 17 levels higher then you would have been had you not trained melee.

For example my mage pure will be trained to 99 magic, 99 defence, 99 constitution, and 43 prayer. At this point it will be a level 102, use rune armor with some magic gear, have +0 magic attack and +170 defence against melee and ranged, and will no longer need to be trained any higher as any new skills will not be used by it.
