Oh, you mean the magic medallion. I think you're looking for Quake. That one is in the Dark World. Go northeast to the Dark World equivalent of Zora's Lake (the area in the Light World where the Waterfall of Wishing is).

You can see it in Part 22 of my walkthrough. It's in the Lake of Ill (that's ill) Omen. If you watch Part 23, you'll see a character in a cave that tells you a clue about throwing something in a circle of rocks in the Lake of Ill Omen.

The Flute is in the Haunted Grove. You need to visit the Dark World equivalent of the Haunted Grove first to meet the Flute Boy who gives you the shovel needed to get it. Then go to the grove in the light world and dig and you'll find it.

For swimming, you need the flippers which are sold at Zora's waterfall/lake.

If you type in "flippers" or "shovel" (without the "") in the search field in the videos section of my channel, you'll see the parts of the walkthrough that show how to get these
