You hurdled way too early in the approach, and you had lot of space off the end of the board. You want 3 inches max from your toes to the end of the board when you hurdle. Also, your leaning way too far forward off the board. Cordinate arm circles with your hurdle movements, to give you a lot of height. Then, once your at least 2 feet in the air, throw into a tuck for your flip with your chin to your chest. :)

@M1G34 Sony DSC-10 with a waterproof case. I use it for Scuba diving, but I decided to try it out in the local swimming pool.
