hey!! nyc vedio..u can also lyk remember general physical exam by a pnemonic- clipjes( cyanosis, clubbing, lymphadenopathy, icterus, pallor, jvp, edema, skin changes..
You also need to comment on: Pulse rhythm, volume and character, You have to check JVP at eye level with the patients sternal angle of louis, eyes- Mucus membranes, Miosis- for signs of Horner's syndrome, Nose- Rhinorrhoea, Nasal septal deviation, hyperplasia of the inferior turbinates, nasal polyps. Your technique for checking chest wall expansion is incorrect. When auscultating the back, ask the patient to cross their arms over their body to get the scapula out of the way.
@emmanuelmarkfrancis The various things you need to include in the examination differs greatly between hospitals and medical schools. These videos therefore should always be taken as a general framework which you can adapt to suit what is common practice in your medical school/hospital. I would disagree that the method for check chest wall expansion was wrong, but I agree that getting the patient to cross their arms can aid in auscultation, although it's not an absolute necessity.
very good video, but ive been told to make sure the head is totally rested when assessing the JVP, because it helps to relax the SCM, which when contracted covers the waveforms
are there any other videos on diferrential RS exam on different pathologies like COPD,PNEUMONIA,tension pnemothorax and others in which your examination and findings are confined to that specific disease????but overall was awesome...
@143poxy We would actually love to do this, however involving "real" patients is very complicated legally and therefore I have opted to only use my close friends who are also students.
Good job Guys! I think this a very instructive video. For OSCE purpose it has to be a little bit faster, at least in Canada where OSCE stations are 10 min including history and physical exam. Thanks and please keep uploading new videos.
Excellent video. i am training to be a nurse practitioner and i have my exams in 2wks and found this very good. good work. excellent noise elicited on percussion!!
@s5432101 you can either do vocal resonance (like in the video) or tactile vocal fremitus. Both test the same kind of thing so you don't need to do both. Thanks for the feedback :)
@geekymedics123 My understanding was that vocal fremitus is regarded as a bit old hat, and it is very rare to see a clinician performing it. Certainly in my (limited) experience I haven't seen this method performed outside of clinical skills tutorials.
@MrWomperstomper Yes vocal resonance is the main one done in clinical practice these days. But in an OSCE I don't think you would be marked down for carrying out tactile vocal fremitus. Thanks for the feedback :)
