Actually, it's the shaft. I had a very similar issue. I believe that the steering shaft is built to collapse during a severe impact to protect the driver. (Usually two shafts connected) Over time there starts to be a little bit of play. Take your car to a mechanic and have them inspect the shaft. Either tighten it up, or put a small tack weld or two where the shafts meet. I hope this helps...

the bushings on your control arm probly has to be replaced.

no, inner tie rods too - it's the rack. still, been fine for 30k miles since this or so.

Steering racks are shit. slightest bit of wear and they are so noisy when goin over bumps at slow speed. Just not durable enough.
