@kmz365 Actually, only one crew messed up. The other crew was just in the wrong place at the wrong time and were unable to do anything to avoid the collision.
They were lined for the siding with a "diverging clear" and the signal dropped due to the oncoming train passing a "stop" signal, and occupying the block.
chriss95367: I (frmr RR supervisor) don't know what the current rules for backing a train are; when had caboose, a crew person could tell engr that movement was safe (may have been a road crossing, people near track, etc.) No caboose = no one to signal back-up is SAFE. Can't move a train until/ unless it's VERIFIED to be SAFE.
the video train engineer had an Approach indication (be prepared to STOP BEFORE passing the Next Signal (same as other engr. did). S/he should have stopped BEFORE passing the red signal at the entrance to the siding; that was sufficient warning to be prepared to stop, and this is a Textbook case of WHY.
@amarkanders777 That was a good guess, but unfortunately wrong. The train crew with the camera had an 'Approach Medium' signal (yellow over yellow), followed by a 'Diverging Clear' signal (red over green). They were lined by the dispatcher INTO the siding. The OTHER train crew disobeyed a 'Stop' signal & fouled the siding switch; causing this collision. 'Google': HQ-2006-48 for the entire NTSB / FRA accident report.
@amarkanders777 I would say that 'wavehopper100' is right. This train was lined into the siding. You can't be lined into the siding with a 'stop' signal. This is a "Textbook case" of the other train 'running' a 'stop' signal.
@1234567891braden Looks like they did jump out, but one died when struck by derailing equipment. Here's the complete NTSB report on the accident: tinyurl dot com/3kste3c

The opposite Engineer did not read his iron. Not only that, but that other train would have split the switch, and quite possibly have derailed. The video train was NOT at fault.
every train should have a gizmo that shows the track in front of them for the next 2km, there is no excuse for this stuff to happen in the modern era.
@gohomepop Stopping distance depends on the speed of the train and the weight it is hauling. A 50mph 15000 ton train is going to take longer than the same train doing 20 or the same train doing 5. Saying a train takes at least a km to stop is a broad and very often erroneous statement.
@VidsbyJrr Suggestion for you. Before you call people idiots you should understand what you are talking about. The crew in the train with the camera were not idiots, did nothing wrong, and were doing their job safely and by the rules.
might sound a bit daft but if the train the camera was mounted on/in hadn't put his brakes on so much wouldn't he have made it into the siding more and the crash wouldn't have been as bad?
@Jamiewaldie1992 Even better yet.If the oncoming train had stopped and waited back at the other end of the siding like it was supposed to do,then there would not have been a crash.
@apollo8724 No.Crews are trained to stay in the cab.The cab is built with a reinforced roll cage construction. If you jump and run you risk geting crushed by the locomotive and freight cars. And no,you cannot outrun the derailing wreckage. It's possible to break a leg jumping from a moving train. And you can't get out the way with a broken leg!!Better to stay in the cab.
@BuchananZ34 No you can't.The camera and microphone are on the outside of the locomotive. That sound is either a flange squeal or a brake valve hiss.Otherwise we would be hearing the crews conversations and radio transmissions.
Clarendon, Texas, United States: A BNSF Railway coal train exceeds its movement authority under an "after-arrival" track warrant while the engineer is making a personal cell phone call, and collides head-on with a BNSF intermodal train. All four crew members jump from their trains before impact and are injured, the intermodal engineer fatally.
They do head on crash tests for these much like on cars. This is a worst case scenario where the switch track distance was not taken into account and the other train did not come to a full stop in time. This is very rare, and is likely operator error, but mechanical failures can cause this too. A full investigation is critical. I cannot find any public evidence in a quick search. If I remember, I will search deeper later.
@NoahLDiamond It was a crew error attributed to the crew of the oncoming train (the one without the camera). No mechanical faults. The full NTSB investigation is available on-line. You can find the NTSB link or investigation number elsewhere in the threads here.

Right. The easiest way to clear up who was at fault:
At 0:38 you can clearly see the signal for the right hand line was GREEN.
The signal changed to RED only about a second before the loco actually passed it.
The points were set AGAINST the other train meaning they were set for the VIDEO one.
Yes they both technically went though a red light but the video train had no time at all to react to it due to the fact it was actually green right up to 0:39 and he passed it at 0:42.
@TurbinePower69 Well the simple answer is the other train went though a red. The signals detected the other train crossing onto the points and realised it wasen't clear and snapped to red at 0:39. All too late for driver recieving the red to respond to it.
@TurbinePower69 The lights change as the locomotive hits it, the switch was just about 20 feet away. the train comming toward had no brakes. Its on some railroad accident history website i cant remember the name. the enineer conductor and fireman were on the runaway. they baled.
@thebro14 Based on what?Look at" BNSF Derailment Locos" and see the aftermath of this "Fake"crash.Google HQ-2006-48 for the NTSB report on this "Fake" crash at Kismet Ca. When all is saidTruth is in jest.A man hears what he wants. And discards the rest.
Most people would just say that they weren't paying attention.Given that cemtexx's statement was about sleep,it was easy to assume that you meant the same.I agree with you about all the rest that you stated.Human beings being human.Rules and regs notwithstanding.
@cmilla646 "They" do have the technical capability to make this investment. But the financial capability is something else. Railroads are going to be required to have PTC on high density mainlines, those carrying passengers, or those carrying TIH by 2015. As to who is going to pay for that....the jury is still out
@Boss302fan If they were sleep, then they would have missed hitting the alerter. And it would have stopped the train. They were fully rested before the start of their shift.
@gebass6 I did not say they were asleep. Never said they weren't rested. I said their eyes were shut. Another way of saying this crew was not alert and following rules.
Even if they missed signals due to "sunshine" the rules cover what to do in such situations. Especially multiple ones which occured in this case.
Their actions, as recorded by the consist's event recorder, clearly indicates this crew and not signals, braking or other outside influences were at fault.
@cmilla646 Maybe. But in this situation the second he saw a "possible concern" was when the signal dropped to red.At that point GPS,calling the dispatcher, calling the other train, would have brought them to the exact same ending. They are still experimenting with PTS(positive train separation)technology to keep this from happening, but the best solution to this is still to just pay attention to signals.
@mpwelk what's blowing the horns gunna do? These aren't cars, they dont stop on the dime. Im sure the drivers in this train and the other one ran to the bacc of the train as soon as they saw the other one
@umahuma4 The Problem wasn't a "Switch Operator", but the opposing (other) train. The train the camera footage came from was to take the diverging route, as evidenced by the first signals which were an "Approach Medium" (yellow/yellow) on BNSF. The next signal, the control point at the signal showed a "Diverging Clear" (red over Green) meaning that the train's route into the siding was clear. at the last second you see the signal drop to Stop as the opposing train slid past into the way.
@Boss302fan The idea of a switch operator likely comes from watching too many train disaster movies that show a "switch operator".As if there was a man standing at every switch ready to throw it like some old steam era rail line. This goes to show that the majority of peoples railroad knowledge is SEVERLY outdated!
@coolguy676 Not at all. They did their job and the oncoming train had the signals to stop and the track lined against them. The crew in the opposing train ignored those signals.
@Chyrnobil The microphone is mounted outside the locomotive cab, so you don't hear crew voices, but that's certainly the train saying "Oh Shit" if I ever heard it!
@brunstj Actually the train that appears at 0:30 was not supposed to even be there. He was to stop about a mile or so back and wait until the camera train had gotten into the siding before proceeding.
@brunstj I'm not giving my opinion.I'm quoting the NTSB report.Google HQ-2006-48.It states that the oncoming train was supposed to stop and wait.It wasn't behind. It was ahead. It wasn't supposed to be there. The camera view train was not at fault.

Is this the Madera Train Wreck?
himfan2215 1 year ago
clearly the opposite engine has run past a red signal and is foul of the adjacent loop road.

@gmspeedfreak the RTC and CTC Screwed up.

@coolguy676 agreed
coolman1558 1 year ago
@coolguy676 Not at all. They did their job and the oncoming train had the signals to stop and the track lined against them. The crew in the opposing train ignored those signals.

the locomotive (with camera) was taken out of service and replaced with the same number
MrCSXboy98 1 year ago
is it just me or can you hear somebody say "shit" at :44?
Chyrnobil 1 year ago
@Chyrnobil The microphone is mounted outside the locomotive cab, so you don't hear crew voices, but that's certainly the train saying "Oh Shit" if I ever heard it!

some one was behind there time
brunstj 2 years ago 16
@brunstj Actually the train that appears at 0:30 was not supposed to even be there. He was to stop about a mile or so back and wait until the camera train had gotten into the siding before proceeding.
gebass6 1 year ago
@gebass6 but u have the fact one could be behind and they both cross the signal light at the same time and never c it go red
brunstj 1 year ago
@brunstj I'm not giving my opinion.I'm quoting the NTSB report.Google HQ-2006-48.It states that the oncoming train was supposed to stop and wait.It wasn't behind. It was ahead. It wasn't supposed to be there. The camera view train was not at fault.
