I've met a lot of male to female transexuals in the process; some who imitates women with visible effort, and some who seem to be natural. It is my belief that those who imitate without much effort have the brain sex of an opposite gender. This is also true for female to male transexuals. Those I've met act just the same as the a guy while they were born with a female genitalia.
