It's implied that Sanae is wholly and completely devoted to Kanako and Suwako, and so when they decied to come to Gensokyo so they could gather faith (and more importantly, survive because the lack of faith was slowly making them fade), she came with them.

Though Suwako doesn't need much faith to survive anymore, which technically makes her an anomaly within the rules of Touhou (Deities need faith, Suwako basically doesn't, or at least, needs very little).

As far as Sanae's backstory goes, I think it has something to do with Kanako, the god of her shrine, wanting to go to Gensokyo to attract more followers, because humans in the outside world were distancing themselves from religion or something.

I think the story is that Kanako (and Suwako, I guess, I'm not sure when the whole I'M A BETTER GOD thing took place) was a god in the outside world, but as technology developed, they stopped believing in her, so she moved her shrine into Gensokyo. And Sanae just came along.

Oh, and that presumably took place in 2007, since that's when MoF was released. So that's that.
