I finally watched this movie and I really think without a doubt that Mike was the product of incest. Mike learns from Richard( the "brother") that his mother spent time in a institution because she was "unsafe". My guess is she was sent there after it was discovered that her pregnancy was the result of her sexually abusing her eldest son Richard. The way I took it was that Mike had been brought up to think his dad was actually his brother, age difference maybe, I haven't seen Idaho in a while so I can't be sure until I watch it again, but Mike knew it was his father. So I think it was his dad, only Mike'd been told it was his brother growing up.
