@NoSleepTilDetox: looks like a coal shute. looks like a hammer or a finger of some sort. Machine was probably being used to clear jams in the mouth of the shute and got too close to the top of the edge and fell in. If the guy survived hes damn lucky looks like the impact broke the whole final drive assy off on side of the undercarrage. Not a fun ride.

Last picture didn't look very nice :( 200 meters from my ex job, a construction crane fell off from it's rails, because end limiters hadn't been mounted or tightened. Crane collapsed towards 5 floors apartment building's balcony, it was constructing. Operator died crushed between the balcony and destroyed operator cabin. :(
I work on VCE (volvo construction equipment) all day everyday and i think this type of machinery is great i cant wait to get to work each day to see what i have to work on next !!!!

Appearantly the hoisting equipment malfunctioned and snapped a ling?¿ (not sure about the name. woven strand of hoisting equipment like 15ft long)
And dropped this excavator like 25meters down. no1 got hurt. as no1 is allowed down the hold when hoisting ;)
