NPC spawns are not related to a patch of any sorts but to entries located in the database. A Blizz-like database cannot be created until a cache from an actual server has been obtained. This will likely become available during the beta phase which is on the way.

The Nexus has two 5-man instances, Ice Caverns and The Oculus, and a raid which has yet to be named but Malygos is confirmed to be the final boss.

Correct, this one is the Ice Caverns. It is a lvl 70-72 instance. The Oculus is going to be a lvl 80 instance. The Oculus instance takes place on the upper rings of the Nexus. As for the raid instance, as of patch 8471/8472, Blizzard has yet to design much more of it than the atmosphere (which looks really cool actually).

poor malygos, more blue dragons are going to be slaughered ... i think at the end of this is will either A. suicide B.die from the player hands or alexstraza hands C. Give his power to another blue dragon and either die/kill himself.

To get the client, no. To log onto the actual blizzard realms, yes. It is the Friend and Family Alpha.
