I accidentally pushed the alarm button in the elevator in my church once. Luckily it was during a boy scout meeting and it was old enough that it wasn't wired to the phone lines. I still nearly pissed myself thinking i had called 911 by pushing the wrong button. (meant to hit "door open")

Most elevators here have the bell hooked to the phone - so if you hold the alarm for 5 seconds the phone dials

nope.. it rings a bell on top of the cab. you ring the bell until somebody complains about it (hopefully)

Some are, but I haven't encountered such a case. I once rang an alarm by accident at the Sheraton Downtown Columbia once while it was at the lobby. The front desk heard it, but nothing happened. They knew it was an accident.
