@Inghildable "Está" is the usted form, which is a form of polite address. It is used when you are talking to someone older than you, someone that you don't know or someone who you feel you should address more formally. Had Eliot known the woman, or had she been a child etc, it would have been fine to use "estás" (the familiar form), but for the reasons mentioned above, "está" was more appropriate. Hope this helped! Well I having people come up to you and start speaking spanish to me(because I'm hispanic) and I have to stop them and tell them I don't know spanish....then they ask me why I don't know spanish....

In the spanish version carla talks in italian as the patient does, it would be weird everyone talking in spanish, lol. In the spanish version carla is from italy, a liitle bit of adaption because of the language.
