The food/aid that is donated is actually introduced to the market. Officials and the military actually sell the food they receive from donations to the people that can afford it. So the food actually gets around to the people, if they can afford it.
In the market only the elite can get ahold of these goods. Basicly the only city that is being focused on is Pyongyang and nampo every now and then. And plus people can't just get the food from the capitol cause of restricted travel in the country.
